Business credit application template
A free, printable trade credit application for businesses that invoice other businesses. Good terms up front make collection much easier later.
Why a credit application matters for collections
The credit application is often the most important document in a later collection case. A signed application can establish who the customer legally is, the agreed payment terms, and contractual rights such as late charges, collection costs, attorney's fees, venue and a personal guaranty, all of which are much harder to establish after the fact.
Section-by-section notes
- Applicant information: use the exact legal name and entity type (check the Secretary of State record), plus any DBA. You'll need it to sue the right party.
- Ownership and principals: identifies who runs the business and who might sign a guaranty.
- Bank and trade references: lets you check payment history. Your reference checks should comply with any applicable privacy rules.
- Credit requested and payment terms: states the limit you're considering and your standard terms (for example Net 30).
- Terms and conditions: the key collection clauses. Late charges and interest must be lawful in the relevant state, and attorney's-fee and venue clauses are enforceable to different degrees in different states.
- Authorization: permission to obtain business credit reports and contact references. Pulling a personal credit report on a guarantor has extra legal requirements, so get advice before doing it.
- Personal guaranty (optional): makes an individual personally responsible if the business doesn't pay. It should be a clearly separate, separately signed section.
- Signature: signed by an authorized officer, with name, title and date.
Have an attorney adapt the terms to your state and industry before relying on them. If you sell to consumers, different laws apply.

Frequently asked questions
What should a business credit application include?
Legal name and entity details, owners, bank and trade references, the credit requested, terms and conditions (payment terms, late charges, collection costs, venue), an authorization signature and, optionally, a personal guaranty.
Is a personal guaranty enforceable?
Often, if it's clearly drafted and signed by the individual. Enforceability depends on state law and the wording, so have counsel review it.
Can I charge interest on past-due invoices through a credit application?
Generally, agreed late charges and interest are enforceable within the limits of applicable law. Rates above legal limits can be unenforceable.
Is this template legal advice?
